(They do accept out of state checks)
Last time I wrote - 5.00 per certificate
 

For Birth Records Include:
Name at birth (first/middle/last)
Date of birth
Father's name
Mother's maiden name
Your relationship to the person
A return address to mail the record to
A self-addressed stamped envelope
A check or money order for 15.00 made out to "Vital Records Registry"
 

 

Birth records over 100 years old & Death records over 50 years old
